Pietro is the head of the Savastano clan, one of the most powerful and influential clans of the Neapolitan camorra. His power is based on force and violence is the means he uses to silence those who dare to challenge him or question his leadership. Despite the trust and obedience that his right-hand man, Ciro di Marzio, professes to him, something breaks inside Ciro when Pietro decides to sacrifice some of his men with the sole objective of giving a bloody warning to Salvatore Conte, head of a rival clan. When Pietro is sentenced to a long prison sentence, Ciro's code of honor forces him to obey Pietro's orders, no matter what, and he agrees to take care of Imma and Genny, Pietro's wife and son.
