Dr. Alan Feinstone is serving time in a maximum security mental hospital where he was sentenced for the crimes committed in the first film. In a session with his psychiatrist, he takes the opportunity to take him hostage and thus escape from the institution. His wife, Brooke, is alive although she suffers the terrible consequences of the torture to which her husband subjected her: without a tongue and with dental implants to replace the ones he extracted. Frightened by the news of the escape and fearing that he might come back for her, she hires a detective to locate her ex-partner.
